Thanks for the replies. That clears it up. I think I incorrectly used "surge" in my post to refer to the surge minion spawn only. I understand that other surge effects still happen.
So to summarize my new understanding:

When a second surge happens on the same tile, all the surge effects occur (increment page, remove cheese from wheel), but a new minion does not spawn.
