Here's the way I'm seeing it. I have a boss minion being placed outside of a surge. How do I do that? I read page 7, it tells me to apply the ambush rules to the top card. No conditional statements here; mention of surges or type of encounter are absent. Note also that the other placement rules in that section do not mention ambushing. To my mind, that's because regular minion ambushing only happens when the encounter card says so, unlike boss minions.
For a boss arriving from a surge I have a similar argument. That bit on page 7 is the only place explaining boss minion initiative placement so I have to assume I'm meant to follow it in this case as well.
I think we disagree because you consider the mention of ambush on page 7 as a clarification of ambush rules for boss minions. I think if they meant to do that they would have put that in the ambush section and the inclusion on page 7 means that it's a general rule.
I'm a bit confused about some ambush cases. The surge rules don't talk about ambush so I'm not sure if a regular minion surge would ambush or not. The ambush rules discuss things for 'determining initiative order for an ambush encounter', it's unclear if this is meant to include the surge on the encounter card or not.
